Crown Censure issued after soldier dies during training exercise

The Ministry of Defence has been issued with a Crown Censure after a soldier died during a training exercise. The gun barrel of an armoured combat vehicle became stuck, causing the turret to rotate and inflict fatal head injuries.

HSE’s investigation found that:

  • there was no safe system of work for driving the armoured vehicle during the exercise
  • risk assessments had failed to identify the danger associated with turret rotation
