Builders merchant fined £2.2 million after worker killed in conveyor crush

A worker was fatally crushed by a 3-tonne pallet after another worker operated a conveyor not knowing he was inside.

The worker had been tasked with removing plastic packaging from pallets of timber before they could be processed at the mill.

While accessing the packaging inside the conveyor framework, the worker was struck twice by a moving timber pack after another operative, unaware of his presence, started and then reversed the conveyor, causing fatal crush injuries at the scene.

Metal fabrication company sentenced after apprentice injured

A teenage worker was injured after his thumb was crushed by the clamps of a metal cutting guillotine during training at a metal fabrication workshop.

The apprentice had been tasked with cutting sheet metal as part of preparation for welding practice. While making a final cut, his thumb was crushed by the guillotine clamps.

HSE’s investigation found a large gap in the machine bed allowing access to dangerous parts, a risk not identified by the company even after the incident.

Council fined £50,000 after worker seriously injured in lawnmower incident

A worker had been tasked with routine grass cutting in a closed graveyard, using a ride‑on mower on sloping ground.

While operating the mower, the machine slid down a steep bank and overturned, carrying the worker over a retaining wall and onto the road below, causing serious injuries.

HSE’s investigation found the council had failed to adequately assess the risks of using ride‑on mowers on slopes and had not put effective control measures in place.
