Scaffolder suffers multiple injuries in fall captured on CCTV

A scaffolder broke his arm, leg and suffered head lacerations after falling through a roof skylight while installing temporary scaffolding edge protection.

He was on the roof carrying materials when he stepped on a skylight and fell more than 6 metres onto a concrete floor.

Two construction companies have been fined a total of £79,300 after HSE’s investigation found that both had failed to plan, manage and monitor the work being undertaken by the scaffolders at the unit.

Offshore firm fined following death of worker whose body was never recovered

The crewman was killed when he fell through a displaced deck grate and was lost to the North Sea. Wave action during deteriorating weather conditions caused the fixings to fail, displacing the grating.

HSE’s investigation found that:

• a grating panel had not been secured in line with the original equipment manufacturer’s specifications
• subsequent inspections had failed to check the fixings used to hold the grating in place

The company was handed a £267,000 fine, with a victim surcharge bringing the total payable to £287,025.

Shipping firm fined £146,700 after worker seriously injured by forklift truck

The company was fined after the worker was struck by a reversing forklift truck.

The worker suffered multiple injuries, including fractures to his right foot and lower leg.

HSE’s investigation found that the company had failed to ensure adequate separation between pedestrians and vehicles during loading and unloading operations.

Following the investigation, HSE has served an Improvement Notice on the company.
