The prosecution was brought by the Health and Safety Executive (HSE), which found that TfL had failed to ensure pedestrians and vehicles could circulate safely within the bus station. Investigators identified concerns with the layout of a pedestrian crossing situated on a bend where buses entered the station, creating limited visibility and uncertainty over right of way. HSE’s investigation revealed that a similar collision had occurred in 2022 and that CCTV footage captured five near misses in the two days preceding the fatal incident.
