Major transport operator fined £2 million after pedestrian killed
A transport operator has been heavily fined following the death of a woman who was struck by a double-decker bus while using an authorised pedestrian crossing. She suffered fatal injuries and was pronounced dead at the scene.
HSE’s investigation found:
- the crossing was positioned at the apex of a bend, where buses turned into the station
- the operator had failed to ensure pedestrians and vehicles could circulate safely at the bus station and had not conducted a risk assessment
- there were failures to adequately monitor bus drivers operating at the station

Two major companies sentenced after driver killed by falling window
The companies have been fined a combined £1.3 million after a window unit weighing 130kg fell 300 feet from a luxury development and killed a coach driver.
HSE’s investigation found the companies – who were involved in the design of the bespoke windows – failed to properly consider the risk if the bespoke windows were left open.
They had also failed to effectively act after a near-miss one year earlier, when a window fell from the same penthouses and just missed striking 2 workers.

Company fined after worker killed in conveyor crush
An aggregates company has been fined £24,000 after one of its workers was drawn into machinery and suffered fatal crush injuries. The worker was drawn into the compression conveyor through an opening in the guarding.
HSE’s investigation found that the company failed to prevent access to dangerous parts of the machinery.
