Company fined six-figures after worker suffered life threatening injuries

A fish processing company has been fined after a worker suffered multiple fractures and 2 collapsed lungs when entangled in a palletiser machine. The worker had been asked to clear a blockage in the machine area and due a fault with the perimeter safety fence, triggered a sensor which restarted the machine.

HSE’s investigation found that perimeter safety fencing around the machine, designed to isolate its power supply whenever the interlocking safety gate was opened, had become insecure after a panel was damaged.

Company issued with heavy fine after multiple hazardous gas leaks

A chemical company has been fined £267,000 after Health and Safety Executive (HSE) inspectors found a corroded pipe which caused a hazardous gas leak on site.

HSE’s investigation found:

the company had known about the leak for 4 months but had kept operating without taking extra precautions
the leak was not a one-off and inspectors discovered multiple leaks from other pipes on site

Product Recall: Bull and Cygnus Fire Alarm Detection System (FADS)

A product recall has been issued for certain Bull and Cygnus wireless Fire Alarm Detection Systems (FADS) after a manufacturing defect was identified that could affect the performance of fire alarm control equipment. The recall was announced by the Office for Product Safety and Standards (OPSS) on 12 August 2026. [gov.uk], [fia.uk.com] The issue relates
