Transport for London (TfL) has been fined £2 million following the death of Grace Mecaley, a 74-year-old retired nurse who was fatally injured after being struck by a double-decker bus while using a pedestrian crossing at Walthamstow Bus Station in December 2023. [itv.com], [press.hse.gov.uk]
The prosecution was brought by the Health and Safety Executive (HSE), which found that TfL had failed to ensure pedestrians and vehicles could circulate safely within the bus station. Investigators identified concerns with the layout of a pedestrian crossing situated on a bend where buses entered the station, creating limited visibility and uncertainty over right of way. HSE’s investigation revealed that a similar collision had occurred in 2022 and that CCTV footage captured five near misses in the two days preceding the fatal incident. Despite these warning signs, risks associated with the crossing had not been adequately assessed or managed. [press.hse.gov.uk], [itv.com]
TfL pleaded guilty to breaching the Workplace (Health, Safety and Welfare) Regulations 1992 and was fined £2 million, in addition to being ordered to pay £5,081 in costs at Westminster Magistrates’ Court. [press.hse.gov.uk], [itv.com]
HSE Inspector Laurence Goodacre said the tragedy highlights the importance of identifying and addressing foreseeable risks before lives are lost, noting that Grace Mecaley was simply walking home after shopping when the incident occurred. [press.hse.gov.uk], [itv.com]
